Neuroimaging studies have found that adult male psychopaths show reduced engagement of limbic and paralimbic circuitry while making moral judgments. The goal of this study was to investigate whether these findings extend to adolescent males with psychopathic traits. Functional MRI was used to record hemodynamic activity in 111 incarcerated male adolescents while they viewed unpleasant pictures that did or did not depict moral transgressions and rated each on “moral violation severity”. Adolescents were assessed for psychopathic traits using the Psychopathy Checklist-Youth (...) Version (PCL-YV), Kiddie Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (KSADS-PL) Conduct Disorder supplement, and Inventory of Callous and Unemotional Traits-Youth Version (ICU-Y). While viewing pictures depicting moral transgressions, CD scores were negatively correlated with hemodynamic responses in the anterior temporal cortex. Adolescents scoring low on the ICU-Y showed a positive correlation between right amygdala responses and severity of violation ratings; those with high ICU-Y scores showed a negative correlation. While viewing unpleasant pictures with and without moral transgressions, PCL-YV scores were negatively correlated with hemodynamic responses in the left amygdala. Overall, the results are consistent with those previously found in adult male psychopaths, but vary depending on the type of psychopathy assessment. (shrink)

Identification of factors that predict recurrent antisocial behavior is integral to the social sciences, criminal justice procedures, and the effective treatment of high-risk individuals. Here we show that error-related brain activity elicited during performance of an in- hibitory task prospectively predicted subsequent rearrest among adult offenders within 4 y of release (N =96). The odds that an offender with relatively low anterior cingulate activity would be rearrested were approximately double that of an offender with high activity in this region, holding (...) constant other observed risk factors. These results suggest a potential neurocognitive biomarker for persistent antisocial behavior. (shrink)

The paper draws a philosophy of dwelling based on a re-evaluation of the notion of vulnerability, which is interpreted in a non-negative sense as constitutive of human being. In this way it outlines ethical and ontological perspectives by focusing on the highlighted relevance of place.

Abstract: The text considers “places” as a plot of conditions for the possibility for human existence, as interweavings of social, cultural, symbolic and material factors on which existence is nourished and which, at the same time, it transforms. Understanding vulnerability as an ontological characteristic of human beings, “taking care” therefore should be considered as the practical disposition to respond to the “event of place” moving towards an increase in being and good. Moreover, in the sphere of healthcare practice, treatments should (...) be included in this broader perspective. This means that the treatment places become understood in reference to the overall conditions that promote health. The result is also a different self-understanding of medicine. (shrink)

O exame ultrassonográfico é uma ferramenta diagnóstica de grande relevância na rotina veterinária, uma vez que, apresenta ampla difusão entre os atendimentos de animais e permite a avaliação de órgãos e estruturas de forma não invasiva, segura e rápida, auxiliando na determinação de diversas afecções, levando ao tratamento adequado e contribuindo de forma positiva no prognóstico do paciente. Atualmente, em muitos casos, a avaliação testicular não faz parte da rotina ultrassonográfica em exames de ênfase abdominal, devido à sua localização anatômica (...) no escroto, podendo assim levar ao diagnóstico limitado e ao tratamento inadequado do paciente quando na ausência da avaliação dos testículos, posto que diversas são as afecções testiculares que podem acometer cães e que apresentam características ultrassonográficas de fácil detecção. Dentre as principais alterações observadas por meio da ultrassonografia, os casos de orquite, criptorquidismo, torção e também neoformações neoplásicas testiculares correspondem à maior casuística, sendo o método ultrassonográfico um exame com 98% a 100% de sensibilidade e acurácia para diagnóstico destas. Dessa forma, o objetivo deste artigo de revisão bibliográfica é elucidar a importância da avaliação testicular em exames ultrassonográficos de rotina, discorrendo inicialmente sobre os aspectos anatômicos e ultrassonográficos, e posteriormente acerca das afecções mais comumente diagnosticadas na rotina veterinária, apresentando ainda as alterações ultrassonográficas causadas no parênquima testicular. Ultrassonografia. -/- THE IMPORTANCE OF INCLUDING TESTICULAR ASSESSMENT IN THE TWO-DIMENSIONAL ABDOMINAL ULTRASOUND ROUTINE IN DOGS FOR THE DIAGNOSIS OF TESTICULAR DISEASES ABSTRACT: The ultrasound examination is a diagnostic tool of great relevance in the veterinary routine, as it is widespread in the current reality and allows the assessment of organs and structures in a non-invasive, safe and fast way, helping in the diagnosis of various conditions, leading to adequate treatment and positively contributing to the patient’s prognosis. Currently, in many cases, testicular evaluation is not part of the ultrasound evaluation in exams with abdominal emphasis, due to its anatomical location in the scrotum, which can lead to limited diagnosis and inadequate treatment of the patient in the absence of ultrasound evaluation of the testicles, since there are several testicular affections that can affect dogs and that present ultrasonographic characteristics that are easily detected. Among the main alterations observed during ultrasonographic exams, cases of orchitis, cryptorchidism, torsion and also testicular neoplastic neoformations correspond to the largest sample, with the ultrasonographic method being an exam with 98% to 100% sensitivity and accuracy for their diagnosis. Thus, the aim of this literature review article is to elucidate the importance of testicular evaluation in routine ultrasound examinations, initially discussing the anatomical and ultrasound aspects, and later on the most commonly diagnosed conditions in the veterinary routine, also presenting the ultrasound changes caused in the testicular parenchyma. C. da Silva -/- Departamento de Agropecuária – IFPE Campus Belo Jardim -/- Departamento de Zootecnia – UFRPE sede -/- -/- PRINCÍPIOS DA DIGESTÃO DOS ALIMENTOS NOS BEZERROS -/- -/- INTRODUÇÃO -/- Se todos os bezerros pudessem ser criados por suas mães, haveria pouca necessidade de inúmeros livros, artigos e trabalhos, como esse, sobre a criação e o manejo básico desses animais. A maioria das vacas desempenha um ótimo papel (...) na criação dos seus descendentes, desde que sejam tomados os devidos cuidados com relação a alimentação, saúde e a outros aspectos relativos à criação desses neonatos. A essência da pecuária bovina relativa à criação de bezerros é manter esses animais vivos e aptos o suficiente para desempenhar atividades produtivas de grande importância na propriedade. Para tanto, os criadores necessitam entender o desenvolvimento do trato digestivo do bezerro e os conceitos básicos da digestão dos alimentos por ele, e é isso que pretendo aclarar de forma clara e concisa com esse trabalho. -/- -/- 1.1 O TRATO DIGESTIVO DO BEZERRO -/- Um animal adulto necessita de quatro estômagos funcionais para dar-lhe a capacidade de utilizar a ampla gama de alimentos disponíveis. -/- O retículo e o rúmen abrigam milhões de micróbios que fermentam e digerem o material vegetal, em especial às presentes nas forragens. O omaso permite a absorção de água do conteúdo do intestino. O abomaso, ou quarto estômago, é o verdadeiro estômago, comparável ao dos humanos, e permite a digestão ácida dos alimentos. -/- O bezerro muito jovem não desenvolve a capacidade de digerir o pasto, sendo assim o abomaso é o único estômago funcional ao nascer. Tanto os animais recém-nascidos como os adultos têm um intestino delgado funcional que permite a digestão alcalina dos alimentos. -/- A figura 1 ilustra a anatomia dos estômagos e intestino delgado de um bezerro recém-nascido. Esse diagrama esquemático mostra as dimensões relativas dos quatro estômagos, o sulco esofágico, que vai do esôfago até o abomaso, e o esfíncter pilórico ou válvula no fundo do abomaso, que controla a velocidade de movimento do conteúdo do intestino no duodeno. -/- O omaso e o abomaso representam cerca de 70% da capacidade total do estômago no bezerro recém-nascido. Por outro lado, nas vacas adultas, eles compõem apenas 30% da capacidade total do estômago (figura 2). -/- A digestão dos alimentos é auxiliada pela secreção de certos produtos químicos denominados enzimas, que estão presentes nas várias partes do intestino. Por exemplo, os bezerros produzem a enzima renina na parede do abomasal para auxiliar na digestão das proteínas do leite, enquanto a lactase é produzida na parede do duodeno para a digestão do açúcar do leite (lactose). Estas enzimas operam mais eficazmente em diferentes níveis de acidez no conteúdo do intestino, ácido no abomaso e alcalino no duodeno. Para conseguir isso, o bezerro segrega eletrólitos, ou sais minerais, com as enzimas, para mudar o conteúdo do intestino de um tipo para outro. -/- Os produtos finais da digestão dos diferentes componentes dos alimentos são absorvidos através da parede do intestino, mediante as vilosidades intestinais, para a corrente sanguínea, onde são levados para as diferentes partes do corpo para o crescimento e desenvolvimento do animal. -/- -/- 1.2 O BEZERRO ALIMENTADO COM LEITE -/- O Leite ou substituto do leite, quer seja apreendido através de uma teta ou bebido de um balde, é canalizado do esôfago através do sulco esofágico para o abomaso. Este sulco é um pequeno canal na parede do rúmen que é controlado por músculos que permitem que os líquidos sejam diretamente enviados para o abomaso e que não entrem no rúmen. O sulco é ativado em resposta a diferentes estímulos. Funciona bem quando os bezerros amamentam-se através das tetas das mães, mas às vezes não funciona quando bebem de um balde. Essa parece ser uma condição psicológica em resposta aos bezerros separados de suas mães. A maioria dos bezerros podem ser treinados pelo tratador para beber o leite do balde rapidamente e bem, a metodologia empregada é a da persuasão do animal, ao qual o mesmo possa responder positivamente à nova rotina diária e à mãe substituta na forma do criador do bezerro. Quando o leite ou o substituto do leite entra no abomaso, forma um coágulo firme dentro de alguns minutos sob a influência das enzimas renina e pepsina. Este é o mesmo processo envolvido na fabricação de queijo ou junket, usando renina para coagular a proteína do leite. A coagulação do leite retarda a taxa em que flui para fora do abomaso, permitindo assim uma liberação constante de nutrientes alimentares em todo o intestino e, eventualmente, para a corrente sanguínea. Pode levar de 12 a 18 horas para que a coalhada de leite seja totalmente digerida. -/- As enzimas que atuam nas proteínas do leite requerem um ambiente ácido e esse é fornecido pela secreção do ácido clorídrico no abomaso. No entanto, até que a digestão ácida esteja operando de forma eficiente, e isso pode levar até sete dias, a única forma de proteína que pode ser digerida é a caseína. Não há substituto para a caseína no bezerro muito jovem. Os substitutos do leite que contêm outras formas de proteína não podem ser devidamente digeridos até que os bezerros sejam mais velhos. Logo, é necessário muito cuidado para não fornecer substitutos ou sucedâneos que contenham fontes proteicas que não possam ser digeridas pelo estômago do animal, para que não ocasione complicações gastrintestinais. -/- A digestão do leite pode ser melhorada com a inclusão de coalho, que pode ser obtido a partir de fábricas de queijo ou aditivos comerciais de leite de bezerro para a primeira semana ou mais. Esses produtos comerciais podem fornecer ácidos adicionais para reduzir o pH abomasal e incrementar a quantidade de enzimas e bactérias específicas para aumentar a taxa de degradação da coalhada de leite. Tais aditivos são chamados probióticos, na medida em que ajudam nos processos digestivos normais. A pesquisa nem sempre os encontrou para melhorar o desempenho e a saúde dos bezerros, e eles são mais propensos a ser benéficos quando os bezerros estão sofrendo de problemas de saúde. Além disso, a sua eficácia, em termos de custo-benefício é, por vezes, questionada. -/- Qualquer leite de uma alimentação anterior está envolvido neste coágulo recém-formado. As proteínas líquidas de soro de leite e a lactose são rapidamente separadas da coalhada de leite e passam para o abomaso. A gordura do leite contida na coalhada de leite é decomposta por outra enzima, a lipase. Esta é secretado na boca pela saliva e incorporada quando o leite é engolido. A alimentação pelas tetas em vez da alimentação através do balde parece produzir mais saliva e, portanto, mais lipase. A digestão adicional da proteína do leite e da gordura ocorre no duodeno com a ajuda das enzimas produzidas no pâncreas. -/- A lactose, que é rapidamente liberada da coalhada de leite no abomaso, é dividida em glicose e galactose e estas são absorvidas na corrente sanguínea para formar a principal fonte de energia para os bezerros jovens. -/- As gorduras são divididas em ácidos graxos e glicerol para absorção e uso como energia, enquanto as proteínas são divididas em aminoácidos e peptídeos para absorção e uso como fontes de proteína corporal. -/- O amido de cereais, por exemplo, é uma importante fonte de energia em bezerros mais velhos, mas esses animais, nas suas primeiras semanas de vida, não conseguem digerir o amido. -/- O abomaso não é ácido até que o bezerro tenha 1-2 dias de idade e isso apresenta vantagens e desvantagens. A principal vantagem é que as proteínas imunes no colostro não podem ser digeridas nesse compartimento estomacal, por isso são absorvidas na corrente sanguínea na mesma forma quando produzidas pela vaca. Isso garante o seu papel como anticorpos para proteger contra as doenças e infecções. A baixa acidez do conteúdo abomasal no bezerro recém-nascido constitui um risco potencial das bactérias (e provavelmente vírus) tomadas através da boca. Estes não serão mortos pela digestão ácida, sendo assim podem passar para os intestinos, onde podem fazer mal ao bezerro recém-nascido. -/- Todos os bezerros pegam bactérias nos primeiros dias de vida e isso é essencial para o desenvolvimento normal do rúmen (flora microbiana). No entanto, a primeira bactéria a colonizar o intestino também pode causar danos. Desde que o bezerro tenha bebido colostro, os anticorpos maternos podem controlar a propagação dessas bactérias mais nocivas. -/- O bezerro alimentado com leite deve, então, produzir uma digestão ácida no abomaso e uma digestão alcalina no duodeno. Isto é conseguido pela produção de eletrólitos na parede do intestino. -/- Bezerros que sofrem de escoriações devido a distúrbios nutricionais ou infecções bacterianas podem perder grandes quantidades de água e eletrólitos em suas fezes. Estes devem ser reabastecidos como parte do tratamento para as escoriações. -/- O colostro é o primeiro leite produzido por vacas recém-paridas. Além de fornecer nutrientes essenciais para a alimentação animal, fornece anticorpos maternos que permitem a transferência passiva de imunidade contra doenças. As recomendações para a alimentação com colostro serão abordadas brevemente em outro trabalho. -/- -/- 1.3 DESENVOLVIMENTO DO RÚMEN E O PROCESSO DE DESMAME -/- Quando os bezerros são desmamados, o custo da criação diminui acentuadamente. Os custos de alimentação são mais baixos, os insumos de trabalho são reduzidos e a incidência de problemas de saúde é menor. No sentido econômico, faz sentido desmamar os bezerros assim que for razoável. No entanto, o bezerro é forçado a sofrer várias mudanças dramáticas, a saber: -/-  A fonte primária de nutrientes muda de líquido para sólido. -/-  A quantidade de matéria seca que o bezerro recebe é reduzida. -/-  O bezerro deve adaptar-se de um tipo monogástrico a um ruminante de digestão, que inclui a fermentação de alimentos. -/-  Mudanças na habitação e no manejo muitas vezes ocorrem em torno do desmame, o que pode aumentar o estresse. -/- Ao nascer, o rúmen é uma parte pequena e estéril do intestino que, ao desmame deve se tornar o compartimento mais importante dos quatro estômagos. Deve aumentar em tamanho, atividade metabólica interna e fluxo sanguíneo externo. Os cinco requisitos para o desenvolvimento ruminal são: -/-  Estabelecimento de bactérias. -/-  Líquido. -/-  Saída de material (ação muscular). -/-  Capacidade absortiva do tecido. -/-  Substrato para permitir o crescimento bacteriano, tais como minerais reciclados, bem como nutrientes para alimentação. -/- Antes do consumo de alimentos sólidos, as bactérias existentes fermentam o cabelo ingerido, o estrato e o leite que flui do abomaso para o rúmen. A maior parte da água que entra no rúmen provém da água livre (água real não contida no leite ou na solução substituta do leite). O leite contornará o rúmen através do sulco esofágico, enquanto a água livre não. -/- O rúmen se desenvolve a partir de um órgão muito pequeno em bezerros recém-nascidos (1-2 L) para a parte mais importante do intestino (25-30 L) por 3 meses de idade. Ele pode aumentar muito rapidamente durante as primeiras semanas de vida, dado o manejo da alimentação direita. -/- O crescimento do rúmen ocorre apenas sob a influência dos produtos finais da digestão no mesmo, que resultam da fermentação de alimentos sólidos pelos micróbios presentes nesse compartimento. O desenvolvimento ocorre em grande parte através do crescimento das papilas ruminais (figura 5 e 6) na parede ruminal (estruturas semelhantes a folhas na superfície interna), que aumentam a área superficial do rúmen e, portanto, a sua capacidade de absorver estes produtos finais de digestão. Portanto, os concentrados favorecem melhor o desenvolvimento dessas papilas ruminais. Para tanto, é necessário, com um tempo, ir incrementando a dieta sólida gradativamente para que esses animais deixem de consumir o leite das vacas produtoras e possam alimentar-se de ração e pastagens, que é a finalidade da criação de bovinos, ou seja, criar animais com menor custo possível e engordá-los através de ração, mas principalmente de pastagens. -/- A capacidade do rúmen e a ingestão de alimentos sólidos estão intimamente relacionadas. O desenvolvimento do rúmen é muito lento em bezerros alimentados com grandes quantidades de leite. O leite satisfaz seus apetites para que eles não tenham fome suficiente para comer qualquer alimento sólido. Logo, é necessário diminuir paulatinamente o fornecedor do leite para esses animais, favorecendo a ingestão de alimentos sólidos e desenvolvendo o rúmen, estômago que digere as fibras das forragens e que os tona animais peculiares. -/- A ruminação pode ocorrer com cerca de 2 semanas de idade e é uma boa indicação de que o rúmen está se desenvolvendo. Alimentos sólidos, bem como a ruminação, estimulam a produção de saliva e isso fornece nutrientes como ureia e bicarbonato de sódio para produzir os substratos para o crescimento e desenvolvimento da flora bacteriana. -/- No desmame precoce, é importante limitar a quantidade de leite oferecida e a sua disponibilidade durante todo o dia. Também é essencial fornecer alimentos sólidos. Os grosseiros (de baixa ou alta qualidade) devem ser oferecidos em combinação com concentrados de alta qualidade. -/- O criador deve tomar cuidado com o fornecimento de alimentos como a ração farelada, uma vez que essa ração pode entrar pelas vias nasais e ocasionar complicações respiratórias; há alguns relatos de bezerros mortos após a ingestão de ração farelada, na autópsia de um dos casos foi diagnosticada uma morte por esses grãos farelados presentes no pulmão. Portanto, para evitar complicações o ideal é que se forneça uma ração denominada peletizada. -/- Os alimentos grosseiros (volumosos) estimulam o desenvolvimento do rúmen, enquanto os concentrados fornecem nutrientes para a alimentação animal que não são fornecidos pelas quantidades limitadas de leite oferecidas. Sem os concentrados, o crescimento dos bezerros é lento, mas o rúmen ainda se desenvolve, resultando em animais barrigudos. -/- A ureia fornece nitrogênio para os micróbios, enquanto o bicarbonato de sódio atua como um tampão ruminal, ajudando a manter um pH estável no conteúdo do rúmen. Isso é, particularmente, importante quando os bezerros comem grandes quantidades de grãos de cereais na vida adulta, pois os micróbios do rúmen podem produzir muito ácido lático durante a fermentação desse material. -/- Envenenamento por grãos ou acidose ocorre quando os níveis de ácido lático são excessivamente elevados e tornam-se tóxicos para os micróbios do rúmen e, eventualmente, para o animal. Assim como os produtos finais que são absorvidos através da parede do rúmen, a fermentação microbiana produz os gases dióxido de carbono e metano e estes são normalmente exalados. Quando algo impede a fuga destes gases do rúmen, o inchaço pode se manifestar em qualquer fase da vida, diz-se do animal estufado. -/- -/- 1.4 O PAPEL DA FORRAGEM NO PROCESSO DE DESMAME -/- Existem inúmeras controvérsias acerca do papel da forragem no processo de desmame. Pesquisas realizadas na década de 1980 indicaram claramente que a forragem era benéfica, enquanto pesquisas da década de 1990 descobriram que nem sempre era necessário o fornecimento da forragem para que os bezerros fossem desmamados e que não sofresse estresse e nem apresentasse baixa eficiência produtiva e reprodutiva no futuro. Na maioria das pesquisas anteriores, os bezerros eram desmamados com a oferta de concentrados moídos como pellets com ou sem a presença de feno longo ou palha. Porém, a inclusão de forragens na dieta melhorou a ingestão e o desempenho, além de permitir o desmame precoce. -/- Na pesquisa posterior, os bezerros geralmente alimentavam-se de concentrados grosseiramente moídos, além de alguns volumosos, enquanto alguns até incluíam volumosos picados finos na mistura (às vezes chamados de mistura muesli por apresentar o capim e a ração). Nesses estudos, verificou-se que a inclusão de feno ou palha adicional teve pouco efeito no desempenho pré-desmame. -/- Os sistemas de criação de bezerros australianos frequentemente diferem dos demais, especialmente em áreas de parto sazonais. Um número excessivo elevado de bezerros precisam ser criados de uma só vez para lhes fornecer, a todos, currais individuais durante todo o seu período de amamentação. Consequentemente, os bezerros são criados em grupos. Além disso, os ingredientes da maioria dos concentrados para esses animais são finamente moídos. Nessas situações, descobriu-se que a palha limpa é um alimento útil e que deve ser incluso no período pré-desmame. Alguns agricultores preferem feno de boa qualidade, porém estes agricultores geralmente têm bezerros em grupos muito pequenos, muitas vezes um ou dois, por isso possuem maior controle sobre a ingestão de forragem. -/- Figura 7: A alimentação volumosa de bezerros alimentados com leite é uma questão controversa. Imagem cedida pelo IPA -/- É difícil e, portanto, mais caro que os produtores de rações incorporem feno picado nas refeições dos bezerros. Os pellets são muito mais fáceis, pois eles vão fluir para silos. Por fim, os produtores de leite devem incluir o componente volumoso no regime alimentar pré-desmame. A pastagem não é a fonte ideal de forragem volumosa para os bezerros alimentados com leite, uma vez que possui muito pouca fibra e uma baixa densidade de energia alimentar. O seu elevado teor de água limita a sua capacidade de fornecer energia alimentar adequada aos animais em crescimento. Para ingerir a pastagem a capacidade ruminal deveria ser maior, bezerros jovens simplesmente não conseguem comer pasto suficiente, a menos que seja de alta em qualidade. -/- Os bezerros criados com leite e fornecimento gradual de concentrados apresentam uma boa função ruminal às 3 semanas de vida, além de possuírem uma suficiente capacidade ruminal para o desmame entre às 4-6 semanas de idade. No entanto, se a dieta for baseada em leite restrito e pastagem de alta qualidade, a capacidade do rúmen pode não ser suficiente para o desmame até às 8-10 semanas de vida. Mesmo assim, as taxas de crescimento seriam menores em bezerros desmamados com base alimentar somente à pasto, uma vez que a ingestão de energia é insuficiente devido às limitações físicas da capacidade ruminal. -/- Se a qualidade for muito boa e produza apetite, os bezerros preferem volumosos ao invés do concentrado, o que leva a uma ingestão reduzida de nutrientes durante alimentação, tendo como efeito um crescimento mais lento. Quando volumosos e concentrados fornecem apetite no animal, juntamente com o leite limitado, os bezerros podem comer cerca de 10% de palha e 90% de concentrados. I investigate the reciprocal relationship between social accounts of knowledge production and efforts to increase the representation of women and some minorities in the academy. In particular, I consider the extent to which feminist social epistemologies such as Helen Longino’s critical contextual empiricism can be employed to argue that it is in researchers’ epistemic interests to take active steps to increase gender diversity. As it stands, critical contextual empiricism does not provide enough resources to succeed at this task. However, considering (...) this view through an employment equity lens highlights areas where such theories need to be further developed. I argue that views such as Longino’s ought to attend to nuances of community structure and cultural features that inhibit critical social interactions, if we are to maximize the epistemic as well as the ethical improvements associated with a social approach to knowing. These developments advance these epistemic theories for their own sake. They also help develop these theories into a tool that can be used by those calling for increased diversity in the academy. El Foro Global de Bioética en Investigación (GFBR por sus siglas en inglés) se reunió el 3 y 4 de noviembre en Buenos Aires, Argentina, con el objetivo de discutir la ética de la investigación con mujeres embarazadas. El GFBR es una plataforma mundial que congrega a actores clave con el objetivo de promover la investigación realizada de manera ética, fortalecer la ética de la investigación en salud, particularmente en países de ingresos bajos y medios, y promover colaboración entre países (...) del norte y del sur.a Los participantes en el GFBR provenientes de Latinoamérica incluyeron a eticistas, investigadores, miembros de comités de ética y representantes de autoridades sanitarias provenientes de Argentina, Brasil, Chile, Colombia, Ecuador, El Salvador, Guatemala, Honduras, Panamá, Perú, Nicaragua y la República Dominicana. Una legítima preocupación por la protección de las mujeres embarazadas y sus embriones o fetos ha llevado a la mayoría de los países de la Región de las Américas a limitar la realización de estudios con mujeres embarazadas exclusivamente a aquellos estudios específicos sobre el embarazo, y a requerir la exclusión sistemática de las mujeres embarazadas o de las mujeres que quedan embarazadas en el curso del estudio. Ciertamente, a lo largo de la historia de la ética de la investigación, se ha creído erróneamente que proteger a una población es sinónimo de excluirla de los estudios. Se sabe ahora que proceder así implica exponer a riesgos mucho mayores a la población que se busca proteger. El embarazo implica cambios fisiológicos sustantivos e impacta profundamente la manera como el cuerpo metaboliza los medicamentos. Sin embargo, por evitar hacer investigación con mujeres embarazadas, no se ha producido la evidencia científica necesaria para tomar decisiones sobre tratamientos e intervenciones preventivas con dosis eficaces y seguras para ellas y sus embriones o fetos. A manera de ilustración, en el 2001 había en los Estados Unidos apenas más de una docena de medicamentos aprobados para uso en el embarazo (1) y en el 2011 la Food and Drug Administration (FDA) aprobó por primera vez en 15 años un medicamento para su uso en el embarazo (2). Como consecuencia de no haber producido la evidencia necesaria, se pone en riesgo la salud de las mujeres embarazadas cada vez que se les da atención médica. Las mujeres embarazadas se enferman y las mujeres enfermas se embarazan, y no se sabe si los medicamentos que se les da son eficaces o siquiera seguros para ellas y sus embriones o fetos. (shrink)

Are there such things as moral truths? How do we know what we should do? And does it matter? Constructivism states that moral truths are neither invented nor discovered, but rather are constructed by rational agents in order to solve practical problems. While constructivism has become the focus of many philosophical debates in normative ethics, meta-ethics and action theory, its importance is still to be fully appreciated. These new essays written by leading scholars define and assess this new approach in (...) ethics, addressing such questions as the nature of constructivism, how constructivism improves our understanding of moral obligations, how it accounts for the development of normative practices, whether moral truths change over time, and many other topics. This paper provides an argument for a more socially relevant philosophy of science (SRPOS). Our aims in this paper are to characterize this body of work in philosophy of science, to argue for its importance, and to demonstrate that there are significant opportunities for philosophy of science to engage with and support this type of research. The impetus of this project was a keen sense of missed opportunities for philosophy of science to have a broader social impact. We illustrate various (...) ways in which SRPOS can provide social benefits, as well as benefits to scientific practice and philosophy itself. Also, SRPOS is consistent with some historical and contemporary goals of philosophy of science. We’re calling for an expansion of philosophy of science to include more of this type of work. In order to support this expansion, we characterize philosophy of science as an epistemic community and examine the culture and practices of philosophy of science that can help or hinder research in this area. The text develops philosophical considerations on the “institutional” dimension of marriage. First of all, the meaning of “institution” is problematized, as it is so much disputed and controversially interpreted today. On the one hand, in fact, it is circumscribed to denote a repressive reality— restraining, delaying, even disciplining—considered necessary and rescuing by some scholars, yet harmful and dangerous by others. On the other hand, accentuating its verbal form, “institution” is also understood in terms of movement, as the novelty that results (...) from the act of instituting, as a discontinuity that opens a field of possibilities. Paul Ricoeur considers institutions as part of the ethical tripod, i.e., of the ways through which human beings can flourish. In the context of these divergent understandings, this paper secondly considers the possibility to speak of marriage as an institution and to take marriage rituals as an example both of rite of passage and aggregation rituals. Bourdieu says that the separation achieved in rituals has a “consecrating” effect. Third, the paper questions whether functional and symbolic changes in marriage and marriage rituals can affect their institutional status and problematize their consequences. (shrink)

In 1950 Adorno et al .'s The Authoritarian Personality study warned that American society contained a minority of individuals whose characters made them prone to become fascists in certain circumstances and that this was a danger common to contemporary industrial society. After early acclaim critics argued that the main threat came from left-wing authoritarian individuals. But research in several countries failed to establish their existence. We trace and evaluate this debate, largely defending the original research. Subsequent argument suggested that the (...) concept of authoritarianism was becoming outdated in post-industrial society, a view that we strongly challenge. While defending the diagnosis and purpose of the original research, we conclude by endorsing the argument that authoritarianism is better described in terms of attitude rather than personality. This gives a clearer psychological description of political movements of the far right and offers more direct measures for their reduction. (shrink)

The goal of achieving Universal Health Coverage (UHC) can generally be realized only in stages. Moreover, resource, capacity and political constraints mean governments often face difficult trade-offs on the path to UHC. In a 2014 report, Making fair choices on the path to UHC, the WHO Consultative Group on Equity and Universal Health Coverage articulated principles for making such trade-offs in an equitable manner. We present three case studies which illustrate how these principles can guide practical decision-making. These case studies (...) show how progressive realization of the right to health can be effectively guided by priority-setting principles, including generating the greatest total health gain, priority for the worse off, and financial risk protection. Papers in this special issue were written upon invitation. They were then subjected to the usual refereeing process of the Journal of Pragmatics. While we have attempted to cover almost all important areas in which context is employed as a conceptual apparatus, our coverage is clearly limited in scope. Accordingly, instead of a general updated overview of the use of context in every conceivable specific field (let's say the state-of-the-art of interdisciplinary research on context: a colossal/impossible enterprise!), we will offer (...) the readers of this special issue an introduction to the problems involved in the study of context by way of eight papers encompassing eight different areas. These are (in alphabetical order): artificial intelligence, bilingualism, child development, cognitive science, conversation analysis, neuroscience, philosophy of language, and pragmatics. All parties involved in researching, developing, manufacturing, and distributing COVID-19 vaccines need guidance on their ethical obligations. We focus on pharmaceutical companies' obligations because their capacities to research, develop, manufacture, and distribute vaccines make them uniquely placed for stemming the pandemic. We argue that an ethical approach to COVID-19 vaccine production and distribution should satisfy four uncontroversial principles: optimising vaccine production, including development, testing, and manufacturing; fair distribution; sustainability; and accountability. All parties' obligations should be coordinated and mutually consistent. For (...) instance, companies should not be obligated to provide host countries with additional booster shots at the expense of fulfilling bilateral contracts with countries in which there are surges. Finally, any satisfactory approach should include mechanisms for assurance that all parties are honouring their obligations. This assurance enables countries, pharmaceutical companies, global organisations, and others to verify compliance with the chosen approach and protect ethically compliant stakeholders from being unfairly exploited by unethical behaviour of others. Many spectacular claims about psychopaths are circulated. This contribution aims at providing the reader with the more complex reality of the phenomenon (or phenomena), and to point to issues of particular interest to philosophers working in moral psychology and moral theory. I first discuss the current evidence regarding psychopaths’ deficient empathy and decision-making skills. I then explore what difference it makes to our thinking whether we regard their deficit dimensionally (as involving abilities that are on or off) and whether we (...) focus on primary or secondary psychopathy. My conclusion is that most grand claims about psychopathy settling long-standing debates in moral philosophy and psychology are overblown, but there is much to be learnt from this disorder when it comes to formulating modern theories of moral psychology. Countermeasures for mpox (formerly known as monkeypox), primarily vaccines, have been in limited supply in many countries during outbreaks. Equitable allocation of scarce resources during public health emergencies is a complex challenge. Identifying the objectives and core values for the allocation of mpox countermeasures, using those values to provide guidance for priority groups and prioritisation tiers, and optimising allocation implementation are important. The fundamental values for the allocation of mpox countermeasures are: preventing death and illness; reducing the association between death (...) or illness and unjust disparities; prioritising those who prevent harm or mitigate disparities; recognising contributions to combating an outbreak; and treating similar individuals similarly. Ethically and equitably marshalling available countermeasures requires articulating these fundamental objectives, identifying priority tiers, and recognising trade-offs between prioritising the people at the highest risk of infection and the people at the highest risk of harm if infected. These five values can provide guidance on preferable priority categories for a more ethically sound response and suggest methods for optimising allocation of countermeasures for mpox and other diseases for which countermeasures are in short supply. Properly marshalling available countermeasures will be crucial for future effective and equitable national responses to outbreaks. (shrink)

In a recent issue of Philosophy East and West Douglas Berger defends a new reading of Mūlamadhyamakakārikā XXIV : 18, arguing that most contemporary translators mistranslate the important term prajñaptir upādāya, misreading it as a compound indicating "dependent designation" or something of the sort, instead of taking it simply to mean "this notion, once acquired." He attributes this alleged error, pervasive in modern scholarship, to Candrakīrti, who, Berger correctly notes, argues for the interpretation he rejects.Berger's analysis, and the reading of (...) the text he suggests is grounded on that analysis, is insightful and fascinating, and certainly generates an understanding of Nāgārjuna's enterprise that is welcome .. In this essay I explore the encounter between religion and music via ideas taken from Olivier Messiaen. I first present his categorization of music and the concepts of sound-colour and dazzlement as the 'directional meaning' of music. I then show how Messiaen relates this to the phenomena of natural resonance and afterimages, and based on this, I present-via Goethe and Rudolf Steiner-the notion of an etheric, evanescent or incorporeal matter. This understanding and experience of matter is then brought to bear (...) on music. In this context the notion of a 'religious experience' can be understood as both referring to content and sentiment related to a religious belief, and, in a more general sense of 'religare' (to bind), as a deeper bond with the material-temporal flow of nature. This gives way to a Johannine vision of Messiaen's musical thinking where the creative-sounding Logos or 'word' is both spirit and matter, and where music may represent a potential awakening to this reality. The genetic modification of pigs as a source of transplantable organs is one of several possible solutions to the chronic organ shortage. This paper describes existing ethical tensions in xenotransplantation (XTx) that argue against pursuing it. Recommendations for lifelong infectious disease surveillance and notification of close contacts of recipients are in tension with the rights of human research subjects. Parental/guardian consent for pediatric xenograft recipients is in tension with a child’s right to an open future. Individual consent to transplant is (...) in tension with public health threats that include zoonotic diseases. XTx amplifies concerns about justice in organ transplantation and could exacerbate existing inequities. The prevention of infectious disease in source animals is in tension with the best practices of animal care and animal welfare, requiring isolation, ethologically inappropriate housing, and invasive reproductive procedures that would severely impact the well-being of intelligent social creatures like pigs. It seems natural to choose whether to have a child by reflecting on what it would be like to actually have a child. I argue that this natural approach fails. If you choose to become a parent, and your choice is based on projections about what you think it would be like for you to have a child, your choice is not rational. If you choose to remain childless, and your choice is based upon projections about what you think it (...) would be like for you to have a child, your choice is not rational. This suggests we should reject our ordinary conception of how to make this life-changing decision, and raises general questions about how to rationally approach important life choices. (shrink)

The question I want to explore is whether experience supports an antireductionist ontology of time, that is, whether we should take it to support an ontology that includes a primitive, monadic property of nowness responsible for the special feel of events in the present, and a relation of passage that events instantiate in virtue of literally passing from the future, to the present, and then into the past.

This article proposes that, in line with moral-cosmopolitan theorists, affluent nations have an obligation, founded in justice and not merely altruism or beneficence, to share the responsibility of the burden of public health implementation in low-income contexts. The current Ebola epidemic highlights the fact that countries with under-developed health systems and limited resources cannot cope with a significant and sudden health threat. The link between burden of disease, adverse factors in the social environment and poverty is well established and confirmed (...) by the 2008 World Health Organization (WHO)’s Social Determinants of Health Commission report. Well-resourced nations generally consider that they have some humanitarian obligation to assist where possible, but this obligation is limited. The following questions are considered: Is reliance on the principle of beneficence to address the global disparities in the social determinants of health and life expectancy at birth good enough? Do well-resourced nations have some obligation from justice, which is stronger than from beneficence, and which cannot be as easily cast aside or diminished, to address these issues? In a globalised world, shaped by centuries of historical injustice and where first-world economies are now so intertwined and reliant on third-world labour, beneficence is not a strong enough principle on which to base an obligation to achieve the WHO vision of ‘health equity through action on the social determinants of health’. This report from the WHO Consultative Group on Equity and Universal Health Coverage offers advice on how to make progress fairly towards universal health coverage.

Confucius gave counsel that is notoriously hard to follow: "What you do not wish for yourself, do not impose on others" (Huang 1997: 15.24). People tend to be concerned with themselves and to be indifferent to most others. We are distinct from others so our self-concern does not include them, or so it seems. Were we to realize this distinctness is merely apparent--that our true self includes others--Confucius's counsel would be easier to follow. Concern for our true self would extend (...) concern beyond the narrow selves we appear to be. The neo-Confucians held just such a view. They espoused an identity with the universe and everything in it, arguing that this identity explains a natural concern for everyone and everything, not just for our narrow selves. However, many things in the universe differ from each other, that is, some have qualities others lack. If they are all one and the same thing then that one thing differs from itself. I will suggest that the objection can be answered with some metaphysical innovation. I will address the objection by sketching a theory--call it the theory of aspects--that explains how numerically identical things can differ qualitatively. (shrink)

Philosophers of experiment have acknowledged that experiments are often more than mere hypothesis-tests, once thought to be an experiment's exclusive calling. Drawing on examples from contemporary biology, I make an additional amendment to our understanding of experiment by examining the way that `wide' instrumentation can, for reasons of efficiency, lead scientists away from traditional hypothesis-directed methods of experimentation and towards exploratory methods.

The renaissance of epistemology between the two world wars forms a bridge between early modern and contemporary philosophy of knowledge. This paper traces the resurgence of interest in epistemology at the turn of the century, as a reaction against the nineteenth-century development of Neo-Kantian and Neo-Hegelian idealism, through the interwar renaissance of epistemology, prompted by major advances in mathematics, logic, and physics, and its ultimate transformation from a theory of ideas and judgement into a theory of propositional attitudes, sentences, and (...) meanings. Motivated ignorance is an incentivized absence of knowledge that arises in circumstances of unequal power relations, a self-protective non-knowing which frees individuals from having to reflect on the privileges they have in virtue of membership in a dominant social group. In philosophical discussions, the term “motivated ignorance” gets used interchangeably with “willful ignorance.” In the first half of this paper, using Charles Mills’ (2007) white ignorance as the defining case, I argue that this is a mistake. A significant swath of (...) cases of motivated ignorance are non-willful, or deep, following Rik Peels (2010). But in all cases, benefits accrued to some in virtue of their social position are gained and maintained at the expense of harms to others. In the second half of this paper, I argue that these harms are what ground attributions of culpability in cases of motivated ignorance and drive the normative requirement that the subject know better, so long as the facts in question are ordinarily and easily knowable (in a sense to be specified). Willfulness is not a necessary condition for culpability, even if it is a sufficient one. (shrink)

This discussion revises and extends Jonny Anomaly's ‘public goods’ account of public health ethics in light of recent criticism from Richard Dees. Public goods are goods that are both non-rival and non-excludable. What is significant about such goods is that they are not always provided efficiently by the market. Indeed, the state can sometimes realize efficiency gains either by supplying such goods directly or by compelling private purchase. But public goods are not the only goods that the market may fail (...) to provide efficiently. This point to a way of broadening the public goods account of public health to accommodate Dees' counterexamples, without abandoning its distinctive appeal. On the market failures approach to public health ethics, the role of public health is to correct public health-related market failures of all kinds, so far as possible. The underlying moral commitment is to economic efficiency in the sense of Pareto: if we can re-allocate resources in the economy so as to raise the welfare of some without lowering the welfare of any other, we ought to do so. Since the past will always be such that the year 1961 has the property of being a time in which Obama was born, it seems impossible that could ever be false in a future context of utterance. Is the virtue of the good citizen the same as the virtue of the good man? Aristotle addresses this in Politics 3.4. His answer is twofold. On the one hand, (the account for Difference) they are not the same both because what the citizen’s virtue is depends on the constitution, on what preserves it, and on the role the citizen plays in it, and because the good citizens in the best constitution cannot all be good men, whereas the good man’s (...) virtue is uniform. On the other hand, (the account for Identity) the two virtues are identical in the good men in the best constitution, in which all are good citizens, each possessing the ability for ruling and for being ruled. This nuanced answer can be seen as Aristotle’s synthesis of a Periclean view (contribution to state blots out personal wrongs) and a Socratic view (no good citizen is without justice). Its nuances reflect the extent to which Aristotle’s conceptions of good citizenship and the best constitution accommodate deviations of what is probable from the human ideal set out in his ethical writings. In the present chapter, I will first address three puzzles regarding the nuances of Aristotle’s answer. Second, I will consider one question about its implication: Does Aristotle’s account for Difference turn out to entail something ultra-Periclean: that no one can simultaneously be a good man and a good citizen in any constitution other than the best? The ancient theory of substance and accident is supposed to make sense of complex unities in a way that respects both their unity and their complexity. On Hume’s view such complex unities are only fictitiously unities. This result follows from his thoroughgoing critique of the theory of substance. I will characterize the theory Hume is critiquing as it is presented in Locke, presupposing what Bennett calls the “Leibnizian interpretation.” Locke uses the word ‘substance’ in two senses. Call substance in the (...) first sense “individual substance” and in the second sense “pure substance.” I will discuss the seven main parts of Hume’s view: (I) that we have no idea of pure substance, (II) that there is no complex individual substance, except in a loose sense, (III) that the fiction of complex individual substance arises in a way parallel to that of the fiction of identity through time, and (IV) results in the fiction of pure substance, (V) that simple qualities and perceptions satisfy the definition of individual substance, (VI) that there is no such thing as inherence, and (VII) that there is no such thing as pure substance. Hume’s views on substance are often mentioned without being discussed in detail. Kemp Smith, Stroud, and Garrett, for example, mostly summarize various claims of Hume in the course of expounding on his theory of the idea of personal identity. In contrast, I will attempt to present a systematic treatment of Hume on substance as a refutation of Locke. (shrink)

Citizens in wealthy liberal democracies are typically expected to see to basic needs like food, clothing, and shelter out of their own income, and those without the means to do so usually receive assistance in the form of cash transfers. Things are different with health care. Most liberal societies provide their citizens with health care or health care insurance in kind, either directly from the state or through private insurance companies that are regulated like public utilities. Except perhaps for small (...) co-pays or deductibles, citizens are not expected to see to their health needs out-of-pocket, at least for some basic level of care. A large proportion of the total global burden of disease is caused by emergency medical conditions. Emergency care research is essential to improving emergency medicine but this research can raise some distinctive ethical challenges, especially with regard to (1) standard of care and risk–benefit assessment; (2) blurring of the roles of clinician and researcher; (3) enrolment of populations with intersecting vulnerabilities; (4) fair participant selection; (5) quality of consent; and (6) community engagement. Despite the importance of research to improve emergency (...) care in low-income and middle-income countries (LMICs) and the widely acknowledged ethical challenges, very little has been written on the ethics of emergency care research in LMICs. This paper examines the ethical and regulatory challenges to conducting emergency care research with human participants in LMICs. We outline key challenges, present potential solutions or frameworks for addressing these challenges, and identify gaps. Despite the ethical and regulatory challenges, conducting high-quality, ethical emergency care research in LMICs is possible and it is essential for global health. (shrink)
